Lancashire and South Cumbria ICB support the use of MyWishes for people wishing to create an Advanced Care Plan. MyWishes is a free to use, 'tech for good' platform. We will empower you to write your Last Will & Testament, safeguard your Digital Legacy, plan your Funeral, curate a Bucket List, leave Goodbye Messages and make plans for your future health and social care within an Advance Care Plan.
After creating an account, our interactive forms and tutorials will help you to document your wishes. Once a document has been completed, download and share it with at least one person who you trust.
You can also share certain documents with your GP or someone who might be involved with your care in the future
